Participant Forms

The Cancer Support Center is a provider of mental healthcare services by licensed professionals. Although all of our programs are offered at no cost to participants and insurance will never be billed, we do require all new participants to complete consent and information forms. Participant forms for children, teens, and adults can be completed in person at both of our locations or completed ahead of time via the PDF forms below.
